Diabetic Daily Log Notepad

I made this for my husband when he discovered that he was diabetic. He faithfully fills it out each day. It keeps a record of his blood sugar, his dietary consumption, reminds him to take his pills and allows him to make notes on extraordinary events which could affect his blood sugar - ie being sick, going to a party the night before, etc. when it comes time for his doctor's appointment, he simply takes the pages for the days since his last appointment and the doctor can see everything. His doctor can easily see if the insulin he's taking is too much or too little. I'm hoping it may be useful to other diabetics as well. Patients can use it themselves or doctors can give it to their patients to use
