Medical Excuse Doctor's Sick Note for Patients

This is a template for a doctor's note that excuses the patient for the time of the patient's appointment. The provider can also use the note to say when the patient can return to work or school. You can edit the text to show the name of the doctor or medical practice in a bold font. There's non-bold text for the office address and phone number. Also, blank lines are available to write the patient's name and appointment date and time. All text is black and can be edited or deleted. The notes are white.
