I would stick with this shape for the best image fit. Customise or delete the back. I am posting a few versions making it easy for you to use as templates. On this, you can change the text to Someone's Name, to Dr., Nurse (and I am doing another EMS) .... change From to Love ... Mother and Dad .... change it all other than the front.
