I would stick with this shape for the best image fit. Customise or delete the back. I am posting a few versions making it easy for you to use as templates. On this, you can change the text to Someone's Name as opposed to Dr., EMS (and I am doing another EMS) .... change it all other than the front. I am doing a couple more with room for the recipient's name, etc.; yet, you can make room here or put the initials on the heart or stethoscope.
