This is a template for a patient referral form for a doctor's office. The provider can also use the note to refer the patient to another physician. You can edit the text to show the name of the doctor or medical practice in a bold font. There's non-bold text for the office address and phone number. Also, blank lines are available to write the patient's name, date of birth, the reason for the referral, the name of the other doctor with phone number, and a lines for the referring doctor to sign on and date. All text is black. The notes are white.
