Here is a simple, elegant option for your baby shower book request needs. Header's on each side were rendered in a swirling, elaborate script font. On the front, this reads "Books for Baby" and on the back, "Thank You for Helping Build Baby's Library". All supporting text has been added to template fields so you can change wording, should you wish to - as much or as little as you need to. On the front, I wrote a quirky bit of verse, and a friendly, explanatory message for the back.
